If you have been a former patient with APC, and need to access to your medical records, please read the following.

Authorized requestors can request their medical records from SHORELINE by accessing the website www.shorelinerm.com and selecting the “Records Request” tab on the home page.

 

Patients will have to fill out and complete a HIPAA authorization form to submit a request. When SHORLINE receives the HIPPA form back, they will notify the requestor of the processing fee and send a credit card authorization form.

 

They charge $0.75 per page not to exceed $100.00. Once payment is received, they will send a secure link via email to the requestor who can then access and download the chart, or patients may request a CD mailed to their address, for an additional $10.00 fee.
